In vitro fertilization (IVF) is a simple series of procedures used to help with fertility or prevent genetic problems and assist with the conception of a child.
What is IVF Treatment (In Vitro Fertilization) ?
IVF Treatment (In Vitro Fertilization), commonly known as ‘test-tube baby’, is a procedure in which the egg is fertilized by sperm outside the body. The woman’s eggs are removed from the ovaries and fertilized in a fluid medium in the laboratory. Once fertilized, the fertilized egg is cultured for three to five days and then transferred to the woman’s uterus, in order to establish pregnancy.
What are the steps in IVF?

Step 1: Ovarian Stimulation
The IVF process begins with ovarian stimulation, where medications are used to encourage the ovaries to produce multiple eggs instead of just one, which is typical in a normal menstrual cycle. Monitoring through ultrasounds and blood tests ensures that egg development is optimal.
Step 2: Egg Retrieval
Mature eggs are retrieved through a minimally invasive procedure known as follicular aspiration, ensuring optimal egg quality. This is done under sedation to ensure the patient’s comfort. Egg retrieval involves carefully inserting a fine needle through the vaginal wall to access the ovaries.
Step 3: Fertilization
The eggs are then fertilized in the laboratory. This can be done through traditional IVF, where eggs and sperm are mixed, or through Intracytoplasmic Sperm Injection (ICSI), where a single sperm is injected directly into each egg for fertilization.
Step 4: Embryo Culture
Following fertilization, embryos are nurtured in a specialized laboratory setting. During this time, they are closely monitored to ensure proper development. Embryologists check for embryo quality before proceeding with the next step.
Step 5: Embryo Transfer
Once the embryos have developed, the best-quality embryos are selected for transfer. The embryo transfer is a simple and painless procedure where the selected embryo(s) are placed into the uterus.
Step 6: Pregnancy Test
Approximately 10–14 days after the embryo transfer, a blood test is done to determine if implantation has been successful and if pregnancy has occurred.

WHAT ARE THE RISKS OF IN VITRO FERTILISATION?

Multiple Pregnancies
IVF increases the likelihood of multiple pregnancies (twins, triplets, etc.), particularly when more than one embryo is transferred. Multiple pregnancies carry additional health risks for both the mother and babies, including premature birth and low birth weight.
Ovarian Hyperstimulation Syndrome (OHSS)
In some cases, the medications used for ovarian stimulation can lead to OHSS, a condition where the ovaries become swollen and painful. While rare, severe cases of OHSS may require hospitalization.
Ectopic Pregnancy
An ectopic pregnancy, where the embryo implants outside of the uterus, can occur in a small percentage of IVF cases. This requires medical intervention as it cannot result in a viable pregnancy.
Emotional and Financial Stress

Step 1:
Investigation prior to IVF to make sure your general health is good
Step 2: IVF Stimulation:
IVE stimulation: It would involve daily injections for a period ranging from 6-12 days. Please visit the hospital for injection before 9:30 am in the morning. Sundays the clinic is usually closed but for injections a nurse would be giving it at a particular time, so confirm with the reception regarding the time
Step 3: Ovum pickup or egg collection :It is done under anesthesia and would require you to come prepared for the same (doctor would explain the preparation involved) you need to be in the hospital for about half a day.
Step 4: Embryo transfer:
This would be the last step of the process usually we do ET in the noon after the outpatient. You need to keep your bladder full for the same since it’s done with full bladder. The results of IVF will be known approximately 12-15 days from transfer. It is advisable to refrain from physical or psychological strain during this period. As you know inspite of the best efforts from your side and our side sometime we are faced with unsuccessful attempts. We suggest that you frequently speak to our counselors and doctors and voice your doubts and be clear in your thoughts so we can reach the positive results as soon as possible. Usually one good transfer gives you a chance at pregnancy of 60% at Aspire and this becomes 80% at the end of 3-4 transfers. This probability of pregnancy can be higher in good prognosis patients and can be lower in poor prognosis patients. Doctor would advise you and give you an individualistic probability depending on your existing condition. If one does not reach a pregnancy in 3 ET there are advanced treatments/investigations to be considered like ERA, LAH, parental karyotyping etc., Which can be discussed with the doctor.

She received her ESHRE (European Society of Human Reproduction & Embryology) Certification as a Clinical Embryologist in 2021. She carries over 21 years of experience and her special interests include PCOS, Embryo Testing (invasive and non-invasive) and Embryo Implantation.

Dr. Vanajakshi is an experienced Fertility Specialist with over 15+ years of experience in Gynaecology and infertility treatment. She received her MBBS from AIMS, Mandya in 1998 passing out as a Gold Medalist in Microbiology. She pursued her MRCOG in 2004 and received her DGO in Obstetrics & Gynaecology in 2014 from Vydehi Institute of Medical Science & Research Center followed by the Fellowship in Reproductive Medicine from Rajiv Gandhi University of Health Sciences (RGHUS), Bangalore.
